The Fitness Profile – Elite Test is a comprehensive blood test designed for individuals looking to improve fitness, performance, and recovery. It measures 75+ markers across overall health, fitness, recovery, inflammation, hormones, hydration, muscle recovery, and nutritional status—key areas that influence how your body responds to training and physical stress.

It includes a comprehensive metabolic panel (CMP), complete blood count (CBC), cholesterol (lipid) panel, inflammation markers (high-sensitivity CRP, interleukin-6, immunoglobulin A), recovery and hydration markers (creatine kinase, lactate dehydrogenase, serum osmolality, and electrolytes), hormones (cortisol, testosterone, and estrogen), as well as multiple vitamin and nutrient levels (vitamins D, B12, B9, and iron plus ferritin and transferrin). This Fitness Profile – Elite Test contains tests and test panels within many key areas important to fitness and performance. In addition to providing the metrics, the results report will provide insights into why some of the standard metrics in base panels such as our CMP may be particularly relevant to athletes and any individuals looking to optimize fitness and performance.

Core health and metabolic readiness

The comprehensive metabolic panel (CMP) helps assess overall health, detect early signs of disease, and monitor chronic conditions. It measures 14 components: blood sugar (glucose), calcium, total protein, liver enzymes, bilirubin, blood urea nitrogen, creatinine, albumin, carbon dioxide, and key electrolytes. Abnormal CMP test results may indicate various health conditions, such as developing or active liver or kidney disease, hypertension, or diabetes and may result in additional tests being run on your existing sample, at no extra cost to you. Several metrics within your CMP including BUN and creatinine are liver and muscle waste products, so abnormal levels may indicate recovery strain.

This CBC test measures and evaluates the components of your blood cells, including red blood cells (RBCs), white blood cells (WBCs), and platelets. Red blood cells carry oxygen throughout your body. Abnormal levels can help identify conditions such as anemia, malnutrition, leukemia, and others. White blood cells are an important part of your immune system, and abnormal levels may mean your body is fighting an infection. Platelets help your blood clot normally. The CBC provides detailed measurements on the count, number, size, and other health-related factors relevant to red and white blood cells.

This lipid panel (or lipid profile) blood test measures your cholesterol and triglycerides levels to assess your risk for cardiovascular diseases. The lipid profile test works by measuring total cholesterol, triglycerides, HDL, LDL, cholesterol/HDL ratio, and non-HDL. Regular lipid panel testing plays a key role in maintaining both general and cardiovascular health.

As part of the CMP, your blood glucose is measured. This is the primary sugar found in your bloodstream. The sugar mainly comes from the food you eat and supplies energy to all cells in the body. High glucose levels may increase the risk of diabetes and associated disorders, such as heart disease.

At the time of your appointment, physical measurements such as your height, weight, waist size, and blood pressure are taken to provide you with additional health-related information.

Along with the biometric screening we provide you a health risk assessment, which is a brief survey that will capture family health history and current health and wellness behaviors. Combined with your diagnostic test results, these metrics will provide you with a personalized Health Quotient Score. The Health Quotient Score, developed by Quest, is a numerical representation of your health on a scale from 1 to 100. This score is unique to you and factors in select lab test results, a health risk assessment, and biometric screening. The higher your score, the better your health. You can use this number as your baseline, then develop a plan to improve your health and track your progress over time.

As a result of the health risk assessment, biometric screening and other results, you will receive risk scoring (each rated high, medium, or low) for 12 risk areas as well as a score for your metabolic syndrome risk.

Inflammation, immune load, and stress response

Cortisol is a hormone produced and released by the adrenal gland in response to stress. It regulates the metabolism of energy sources such as carbohydrates, lipids, and proteins in the body, as well as blood glucose levels and immune responses. The body's cortisol levels fluctuate throughout the day, peaking in the morning after waking and decreasing until midnight when they are lowest. Cortisol's main function during exercise is to keep blood glucose levels stable. Furthermore, during activity, cortisol levels rise, which helps the body power through exercise and training. However, cortisol also has a catabolic effect since it stimulates tissue breakdown and hinders protein production.

C-reactive protein (CRP) is a protein produced in the liver. Blood concentrations of CRP typically increase in response to tissue injury, infection, and inflammation. CRP levels can rise as much as 1,000-fold after an acute event, and this test can help identify and monitor acute inflammatory responses. The high-sensitivity CRP (hs-CRP) is more sensitive than a CRP test and accurately detects low but persistent levels of inflammation.

Interleukin-6 (IL-6) is a circulating marker of inflammation in the body. IL-6 is mainly released from the body's immune cells to function as a 'cytokine' in the body. Cytokines are small proteins that immune cells release to help coordinate how cells communicate during an immune response. In response to exercise, skeletal muscle can produce and release IL-6, which may play a role in energy metabolism, including how the body uses glucose and fats, and may support red blood cell production. If IL-6 is high, it may indicate that inflammation or infection is present in the body.

Immunoglobulin A (IgA) is a type of immune protein that can be used to evaluate immune status. IgA is produced by specific cells in the body (called plasma cells) in response to exposure to a harmful foreign substance (like bacteria or a virus) to help fight off an infection. Different types of immunoglobulins play specific roles in defending the body from viruses and bacteria. Specifically, IgA functions to protect the respiratory tract and gastrointestinal tract (stomach and intestines). IgA levels may indicate that the immune system's ability to fight infection is reduced, and the athlete may be more susceptible to an upper respiratory tract infection (or common cold).

Mercury is a heavy metal that may, when elevated in the body, may increase oxidative stress and inflammation, disrupt energy production, affect how your muscles and nerves work together, and contribute to fatigue and slower recovery. The type of mercury measured in this test (organic mercury, via blood) is most commonly linked to dietary sources, especially fish and seafood, as well as some supplements that may contain trace heavy metals.

Hormones and performance signaling

Testosterone is the primary male sex steroid hormone but is also present in women. Its release is regulated by specific parts of the brain, called the hypothalamus and the pituitary gland. In men, the testes produce testosterone. In women, the adrenal gland and the ovaries produce testosterone in smaller quantities. This test measures the total level of testosterone in the body, which is made up of free and bound testosterone. Testosterone's effects on the body are considered anabolic (muscle building) and androgenic (developing and maintaining the male reproductive system). Testosterone is essential for athletes, particularly males, because it helps the body adapt to training and build muscle. Testosterone boosts protein production, decreases protein breakdown, and increases red blood cell production.

Estrogens are a group of hormones primarily secreted from the ovaries in females but are also present in men. They play a prominent role in regulating menstrual and reproductive cycles. Of the types of estrogens, estradiol (often referred to as just estrogen) is the primary sex hormone in females and is most sensitive to acute and chronic changes. Estradiol levels are generally low in men. In women of reproductive age, estrogen concentrations cycle throughout the month, with levels highest at the onset of menstruation. While essential to the female reproductive system, estradiol also significantly affects female musculoskeletal health. Estradiol has been linked to skeletal muscle status and improved strength recovery by modulating inflammation and promoting regeneration after injury.

Muscle recovery and cellular stress

Creatine kinase (CK) is an enzyme in the muscle that is involved in producing energy to fuel muscle actions for short, high-intensity activities like sprinting and jumping. CK can leak from the muscle into the bloodstream when the muscle membrane is disrupted during intense training sessions. The muscle work during training that causes the CK to leak is required for strength, size, and power improvements to occur in the muscle. Elevated levels of CK in the blood may indicate muscle damage or inflammation.

Lactate dehydrogenase (LD) is an enzyme in the muscle involved in metabolism. LD can leak from the muscle cells into the bloodstream when metabolism of glucose is high during exercise or if cells are damaged or destroyed. LD values may indicate the amount of “muscle damage” in response to exercise. This “damage” is the normal response to intense exercise, which is required for muscles to adapt and respond to training. Increases in LD following exercise depend on training time and intensity, with higher levels seen after longer and more strenuous sessions.

Hydration and electrolyte balance

Serum osmolality is a blood test to help individuals engaged in particularly high intensity training monitor the body's water balance. Osmolality is a measure of how concentrated a fluid is. In other words, it is a measure of the concentration of all chemical particles found in the fluid (serum) portion of blood. Low blood levels may indicate overhydration, while high levels may indicate dehydration.

As part of your CMP, calcium is measured. Calcium is one of the most abundant minerals in the human body and plays a key role in multiple bodily functions. Most of the calcium in your body is stored in your bones, with only 1% circulating in your bloodstream. Calcium is required for strong bones and teeth and proper nerve, muscle, and heart function.

As part of your CMP, sodium is measured. Sodium is a vital mineral in the body. It is carefully regulated by the kidneys and plays an important role in the body's water balance. Sodium is an electrolyte, which means it is a mineral with an electrical charge. Electrolytes affect how much water is in your body and how your muscles work. During exercise, sodium is the major electrolyte lost through sweat and the most important one to replace after exercise. Because sodium is essential for muscle and nerve function and hydration, it is critical to monitor and manage sodium levels in the body. Out-of-range levels may decrease muscle power and motor control during performance or cause muscle cramping.

As part of your CMP, potassium is measured. Potassium is an electrolyte that is also one of the body's most important minerals. It helps to regulate water balance, blood pressure, and heart rate. Potassium is also necessary for muscle contraction and nerve function. Monitoring and regulating potassium levels in the body is critically important, as potassium is essential to muscle and nerve function and hydration. Out-of-range levels may decrease muscle power and motor control during performance or lead to muscle cramping.

As part of your CMP, chloride is measured. Chloride is an electrolyte that helps to balance fluids inside and outside of your body's cells. Chloride plays an essential role in metabolism and is instrumental in maintaining proper blood volume, blood pressure, hydration fluid balance, muscle response, and acid/base balance. Blood chloride can be elevated when the body loses excessive amounts of sodium- chloride in the body through high rates of fluid loss. If the body is inadequately hydrated, the athlete may not be able to perform optimally.

Nutritional status and oxygen delivery (performance fuel)

This test measures the amount of 25-hydroxyvitamin D in your bloodstream, an indicator for the level of vitamin D in your body. Vitamin D is involved in regulating calcium and phosphorus, which are essential nutrients to promote bone strength and cellular health. Vitamin D can also help the immune system fight infections and is known to be involved in muscle function and protein production. Your body produces vitamin D after exposure to sunlight or from consuming certain foods and supplements.

Vitamin B12, also known as cobalamin, is an essential nutrient necessary for producing and maintaining nerve and red blood cells, supporting the nervous system, and helping to create DNA, the basis of all cells. Optimal vitamin B12 status is important to support metabolism, red blood cell production, and nerve function. Vitamin B12 must be supplied by the diet or through supplementation because the body is not able to produce it on its own.

Folate, also known as vitamin B9, is required for red and white blood cell development, tissue growth, cell function, DNA production, and the breakdown, usage, and creation of proteins. The term folate is often used to describe the naturally occurring form, while folic acid refers to the supplement form added to foods. Because folate is linked to red blood cell function, deficiencies in folate can lead to decreased endurance and fatigue.

Ferritin is a protein in the blood that contains iron and helps you understand how much iron your body is storing. Blood levels of ferritin correlate to the body's iron status. Because iron is an important component of oxygen-carrying hemoglobin in the blood, low ferritin levels may result in suboptimal physical performance and endurance.

Iron is an essential micronutrient in the body. In the body it plays an important role in the production of healthy red blood cells. It also is a key component of proteins such as hemoglobin (the protein in red blood cells that carry oxygen), myoglobin (the protein in muscle that binds to oxygen), and enzymes (proteins that enable metabolic processes to occur). Iron is needed for proper growth and development. Iron must be supplied by the diet or through supplementation because the body is not able to produce it on its own.

Transferrin is a protein in the blood that binds to iron. Transferrin levels provide a measure of the iron binding capacity in the blood. The amount of transferrin in the bloodstream reflects the blood’s capacity to transport iron, and therefore it is an indirect measure of the body’s iron stores. When iron stores are depleted, the liver produces transferrin at an increased rate in an attempt to increase iron-binding and carrying capacity. Low transferrin may indicate iron deficiency or anemia (a condition where red blood cells are not adequately able to transport oxygen). If transferrin is low, iron binding capacity may be impaired.

How does muscle affect eGFR?

Athletes may have eGFR outside of the reference range due to large amounts of muscle mass and muscle metabolism. Due to increased amounts of creatinine in the blood, eGFR rates in athletes may be lower if the test is performed the day after a high-volume workout. In this situation, the effect is unlikely to be kidney dysfunction, but rather a larger demand on the kidneys because of high metabolic output. eGFR results should be monitored to ensure the kidneys are functioning appropriately to filter waste products in the blood.

What does it mean if my white blood cell count is out-of-range?

A low white blood cell count, also called leukopenia, may mean your body cannot fight off infection, and an excessive white blood cell count (leukocytosis) may indicate an infection or inflammation. Additionally, white blood are a component of the immune system and are classified into these types: neutrophils, monocytes, lymphocytes, and eosinophils.

If the neutrophil count is high, this may be indicative of an acute infection, stress, muscle damage, rheumatoid arthritis, or other inflammatory disorders. If the neutrophil count is low, it may be the result of a bacterial infection or medical condition. A consistently low neutrophil count, in the absence of disease, may be a sign of overtraining.

Monocytes can be elevated because of an infection, inflammation, allergic reaction, or asthma. During exercise, the number of monocytes in circulation increases. However, the number of monocytes in circulation normally returns to resting levels when exercise stops. Athletes, especially endurance and female athletes, may normally have lower monocyte counts than the reference range. This is often considered an adaptation to training.

An elevated lymphocyte count may mean an infection or other immune challenge. If the lymphocyte count is low, it may be the result of a medical condition. A low lymphocyte count may also occur from under-nutrition, stress, use of corticosteroids (such as prednisone), or high exercise volumes.

If the eosinophil count is high, it could be the result of an infection, allergic reaction, eczema, or asthma. Certain medical conditions such as autoimmune diseases, hay fever, or leukemia are also associated with elevated eosinophil counts. If the eosinophil count is low, it may be the result of high levels of cortisol in the body or alcohol intoxication.

Like eosinophils, if basophil numbers climb too high, they can contribute to allergies and other inflammatory reactions in the body. A low basophil count may indicate that the athlete is unlikely experiencing an allergic response and this is generally not a cause for concern.

I thought triglycerides were harmful, so why do they matter for athletic performance?

In medicine, triglycerides are used to monitor for conditions like diabetes or coronary artery disease. However, triglycerides can provide an important fuel during exercise. Triglycerides are a type of fat found in the blood. When you eat, your body converts any calories it doesn't need to use right away into triglycerides. The triglycerides are then stored in your fat cells. Later, when your body needs energy, hormones release the triglycerides into your blood so they can be used for fuel. This process is known as lipolysis.

People who exercise frequently tend to have lower levels of triglycerides in their blood because their bodies use more triglycerides for energy during exercise. Distance runners and other endurance athletes usually have the lowest levels of triglycerides because they use the most energy during exercise. Triglycerides are an important source of energy for athletes, helping them to maintain their performance during exercise.

Does hypothyroidism or hyperthyroidism affect athletic performance?

The primary role of thyroid hormones is to regulate metabolic rate, and improper function of the thyroid gland could lead to significant decreases in athletic performance.

If TSH is high, the thyroid gland is likely not producing enough thyroid hormones or working as it should. Underactive thyroid function can affect energy levels, growth, mood, and performance. Because of fatigue, hypothyroidism can make it difficult to exercise, and your capacity to train as an athlete is significantly reduced.

When the TSH level is low, it means that the thyroid gland is overactive and producing high amounts of thyroid hormones. An overactive thyroid may make maintaining muscle mass difficult. The over secretion of thyroid hormones can have a catabolic effect on skeletal muscle tissue.

Importantly, athletes can maintain healthy thyroid hormone levels and avoid these complications with proper diagnosis and treatment.

What is the relationship between potassium and sodium?

The electrolytes, potassium and sodium, work in tandem to help maintain fluid balance and blood volume, nerve and muscle growth, heart function, and pH balance in the body. Your cells use potassium alongside sodium, and both are essential for cell function. There is a mutual trade-off between sodium and potassium to maintain balance within the body. An ion exchange occurs when sodium or potassium enters a cell. When sodium enters the cell, potassium leaves, and when potassium enters the cell, sodium leaves.

Although commonly used interchangeably, salt and sodium are not the same. Table salt is sodium chloride, a specific type of sodium obtained through the diet rather than produced naturally by the body. However, more than 70 percent of dietary sodium is obtained through packaged and prepared foods, not from table salt added while preparing or eating food.

Aren't iron deficiency and iron deficiency anemia the same?

No, there is a difference between the terms, though they sometimes seem to be used interchangeably. Anemia is caused by insufficient levels of hemoglobin, whereas a lack of iron causes iron deficiency.

It's possible to have an iron deficiency while maintaining a normal hemoglobin level despite your body’s being deficient in iron. However, a person with iron deficiency anemia has neither sufficient levels of hemoglobin nor iron. It's important to note that anemia can also be caused by issues other than iron imbalance, such as blood loss, inflammation, and certain health conditions.
