Quest Consumer Inc. and its successors (“Quest Consumer”)

Quest Consumer Inc. is a member of the Quest Diagnostics family of companies. It operates the questhealth.com website. Quest Consumer Inc. is a subsidiary of Quest Diagnostics, but, unlike Quest Diagnostics, Quest Consumer Inc. is not a laboratory testing company or healthcare provider and is not a covered entity that is subject to the requirements of HIPAA. Quest Consumer Inc.’s successors would include, but not be limited to, any company that takes over the operations of Quest Consumer Inc. in the future.

Service providers

Companies that are contracted with Quest Diagnostics and/or Quest Consumer Inc. to provide services to them or on their behalf. Quest Diagnostics Incorporated and its subsidiaries and affiliates.

Quest Diagnostics Incorporated and the family of companies that it owns or controls.

Some Quest Diagnostics affiliates are CLIA-certified laboratories and HIPAA-covered entities. Other affiliates, such as Quest Consumer, are not.
